Clinical Ethics, Ahead of Print. ObjectiveThe purpose of this study was to assess if a relationship existed between audiologists’ perceptions of moral distress, occupational wellbeing, and patient-practitioner orientation.DesignThe Moral Distress Thermometer, Health and Safety Executive Management Standards Indicator Tool and Patient-Practitioner Orientation Scale was sent out to all audiologists registered with the professional body Audiology Australia.Study sample: A total of 43 audiologists completed the questionnaires.ResultsUsing a multiple linear regression model there was no evidence of a relationship between patient-practitioner orientation and (...) either moral distress or occupational wellbeing. However, an association between higher moral distress and lower occupational wellbeing was demonstrated. The occupational wellbeing subscales of ‘managerial support’, ‘peer support’, ‘relationships’, ‘role’, and ‘change’ were strongly associated with poorer scores on moral distress.ConclusionFor our sample of audiologists, ethical conflict appeared to be linked to reduced job satisfaction. There is a need to consider ways of providing supportive organisational structures in which ethical conflicts can be openly discussed and negotiated in order to improve occupational wellbeing in the profession. The retirement village sector1 is one part of the increasingly marketized `aged-care' services in New Zealand and in many other parts of the industrialized world. While critical researchers have examined organizational and residents' representations of aging, retirement, and retirement communities in the context of `the market', there is no research that examines communication related to residents' enactment of participation within these settings with respect to these processes of marketization. We aim to refine, complicate, and extend what we might call `the (...) marketization thesis' through this study that examines resident participation within retirement communities. This essay is based on a comparative examination of two major New Zealand retirement community organizations. The rhetorical and critical discourse analysis reveals the complexity of what `participation' means for the residents within the context of an institution that is in transition from a medicalized to a marketized model of service delivery. Through a close examination of these meanings, we offer new insights into issues of individual action and freedom within the frame of a market-driven and avowedly `customer-centred' organization and consequently suggest a reconsideration of participation in organizations in which clients are also `insiders'. If, as may be generally agreed upon, the term science is to be taken to mean verified knowledge, then it has three attributes: the logical and methodological; that is, how we arrive at verified knowledge; the epistemic; that is, the bodies of verified knowledge that have been arrived at; and the sociological; that is, the organization of men by means of which the bodies of knowledge have been arrived at and the method prosecuted.

The position that science is a technique establishing the means to achieve any stipulated end has now fanned out and been defended by social scientists as well as by natural scientists. It is the thesis of this paper that the bifurcation of science and morality derives from the social status of both science and scientists today, and involves, wittingly or unwittingly, an uncritical acceptance of dominant social values. t is abundantly evident that rates of evolution vary. They vary greatly from group to group, and even among closely related lineages there may be strikingly different rates. Differences in rates of evolution, and not only divergent evolution at comparable rates, are among the reasons for the great diversity of organisms on the earth. Among the living primates there are, for instance, some rather unspecialized or primitive prosimians (i.e., little changed from Eocene progenitors), a larger number of divergently specialized prosimians, (...) many monkeys of different degrees of progression and divergence, a few apes, and the unique species of man. Important as is the purely divergent evolution, it is also clear that differential rates are involved. At the extremes, the lineages of the more primitive living prosimians have evolved less rapidly as regards the whole of their structure and adaptive position than has the lineage of man. (shrink)

Objective: There is a demonstrated link between the mental health and substance use comorbidities experienced by young adults, however the vast majority of psychological interventions are disorder specific. Novel psychological approaches that adequately acknowledge the psychosocial complexity and transdiagnostic needs of vulnerable young people are urgently needed. A modular skills-based program for emotion regulation and impulse control addresses this gap. The current one armed open trial was designed to evaluate the impact that 12 weeks exposure to ERIC alongside usual care (...) had on young people's ability to regulate emotions, as well as examine potential moderating mechanisms.Methods: Seventy nine young people were enrolled to the 12 week intervention period. Twenty one practitioners from youth and community health services delivered relevant ERIC modules adjunct to usual care. Linear mixed effects regression was used to examine change over time across the primary outcome of emotion dysregulation and secondary outcomes of depression, anxiety, stress, experiential avoidance and mindfulness. Moderation analyses were conducted to test whether the magnitude of change in emotion dysregulation moderated change over time in secondary outcomes.Results: Analyses revealed significant improvement in the primary outcome of emotion dysregulation with a moderate effect size, in addition to decreases in the secondary outcomes of depression, anxiety, stress and experiential avoidance. No improvements in mindfulness were reported. Moderation analyses revealed that the residualised change over time in emotion dysregulation moderated the change over time in symptoms of distress, depression, anxiety, stress, experiential avoidance, and mindfulness.Conclusion: Reductions in the severity of emotion dysregulation, depression, anxiety, stress and experiential avoidance are promising, and were evident despite the complexity of the participants and the diversity of the service setting. The improvements found in each outcome were only observed for those young people whose emotion regulation also improved, providing preliminary evidence for the role of emotion regulation as a key treatment target in this population. Predictive processing accounts of neural function view the brain as a kind of prediction machine that forms models of its environment in order to anticipate the upcoming stream of sensory stimulation. These models are then continuously updated in light of incoming error signals. Predictive processing has offered a powerful new perspective on cognition, action, and perception. In this chapter we apply the insights from predictive processing to the study of emotions. The upshot is a picture of emotion as inseparable from (...) perception and cognition, and a key feature of the embodied mind. (shrink)

Predictive processing has begun to offer new insights into the nature of conscious experience—but the link is not straightforward. A wide variety of systems may be described as predictive machines, raising the question: what differentiates those for which it makes sense to talk about conscious experience? One possible answer lies in the involvement of a higher-order form of prediction error, termed expected free energy. In this paper we explore under what conditions the minimization of this new quantity might underpin conscious (...) experience. Our suggestion is that the minimisation of Expected Free Energy is not in itself sufficient for the occurrence of conscious experience. Instead, it is relevant only insofar as it helps deliver what Ward et al. ( 2011 ) have previously described as a _sense of our own poise over an action space_. Perceptual experience, we will argue, is nothing other than the process that puts current actions in contact with goals and intentions, enabling some creatures to know the space of options that their current situation makes available. This proposal fits with recent work suggesting a deep link between conscious contents and contents computed at an ‘intermediate’ level of processing, apt for controlling action. (shrink)
